How to use this template

Identify risks. Brainstorm with the team and stakeholders; describe each as a cause and its effect.
Score probability and impact. Use a 1–5 scale for each; the template multiplies them into a risk score.
Assign an owner. Every risk needs one named person responsible for watching and acting on it.
Plan mitigation. Decide how to reduce probability or impact, or accept the risk explicitly.
Review regularly. Review the top risks every sprint and update scores, status and actions.

Tips

Focus attention on scores of 12 and above.
Closed risks stay in the register as an audit trail.
